Blair HealthChoices CARES team recovery assistance is about helping people find their path to recovery. Recovery helps people return to a healthy state of mind, body, and spirit. The Blair CARES Team is here to help you do that.

Who is eligible?
If you are enrolled in medical assistance, have been in the hospital or a rehabilitation center, and are having a hard time with your recovery, you may be eligible to work with the Blair CARES Team.
If you are a parent or caregiver and have a child that is struggling in school or at home because of their behavioral health, your child and family may also be eligible to work with the Blair CARES Team.

Who can be part of your Blair CARES Team?
Care Coordinator: Someone who will help to assess your needs and develop and plan for recovery.
Family Care Coordinator: Someone who can help your child and family in assessing your child and family’s needs and help to develop a plan for improving the family’s well-being.
Peer Advocate: Someone who can share their mental health recovery journey and connect you to recovery resources.
Certified Recovery Specialist: Someone that is in recovery from an addiction and who wants to help you if you want to stop using drugs and/or alcohol.
Housing Specialist: Someone who can help find housing, or help you remain in your housing.
Employment Specialist: Someone who can help you find employment or volunteer work that you enjoy.
