Annual Report
Blair HealthChoices thrives on partnerships with other systems, schools, providers, adult consumers, youth and families. In collaboration with our partners, we hope to improve the quality of life for all in Blair County. Blair HealthChoices manages the medical assistance funds for behavioral health services in Blair County and collaborates with our partners to ensure access to services of the highest quality.
Blair HealthChoices Annual Reports: An annual review of the Behavioral HealthChoices Program in Blair County including demographics, expenditures, and quality measures.
CFST Report
We value the voice of the consumer. Blair HealthChoices contracts with The Center for Behavioral Health Data Research, Inc. to conduct satisfaction surveys with consumers of behavioral health services by consumers. The information helps Blair HealthChoices, Community Care and our contract providers of behavioral health services ensure they are providing the highest quality of care and identify opportunities for improvement.
Consumer and Family Satisfaction Survey Quarterly Reports: A quarterly summary of the survey results includes consumer feedback and on their satisfaction with their behavioral health services. These survey results are given to each behavioral health provider to review and incorporate into their quality improvement efforts.
Cultural & Linguistic Resources
A Cultural and Linguistic foundation training provided by Kelsey Leonard, Cultural and Linguistic Competence Coordinator from PA Care Partnership. This training provides Blair County demographics and tools for creating focused initiatives within your agency.
